Submit a Lien for Resolution

This section applies to attorneys and law firms who have already signed an Engagement Agreement with The Lien Project, PC (TLP), and are ready to submit a lien. If you have not yet completed the engagement process, please visit our Ready to Engage page before submitting a case.

Once you have an engagement agreement on file, you are ready to Refer a Case for reduction and resolution. Below is an outline of that intake process. 

Submission of an intake form and supporting documents does not guarantee TLP acceptance of the lien for handling. If you are unsure whether a lien is appropriate for referral, we encourage you to call, email, or schedule a consultation via Calendly prior to submission.

Once your Engagement Agreement is on file, you may refer cases to us at any time. Once a lien referral is accepted, our team will manage every aspect of lien resolution, including verification, negotiation, and final reduction, allowing you to focus on litigation while we focus on protecting and maximizing your client’s net recovery by minimizing lien exposure.

Step 1: Complete the Intake Form

Each lien type requires specific documentation. Intake forms include:

Medicare Liens

    • Completed Medicare Lien Intake Form
    • Conditional Payment Letter (CPL) from Medicare
    • Final Demand Letter (if available)
    • Settlement statement (if available)
    • All correspondence with CMS, BCRC, or CRC

Medi-Cal Liens

    • Completed Medi-Cal Lien Intake Form
    • Notice of Lien from the Department of Health Care Services (DHCS)
    • Relevant medical bills/statements
    • Settlement details (if available)
    • Communications with DHCS

Private Insurance & ERISA Liens

    • Completed Private Insurance / ERISA Lien Intake Form
    • Subrogation or reimbursement notice
    • Health plan documents, including the Summary Plan Description (SPD) if available
    • Correspondence with subrogation firms (e.g., Rawlings, Conduent, Phia, Equian, Optum)
    • Settlement breakdown and related medical expenses

Hospital Liens

    • Completed Hospital Lien Intake Form
    • Hospital lien notice or billing records
    • Itemized statements
    • Communications regarding prior negotiations or disputes
    • Settlement summary or case status

Government Liens (MCRA, TRICARE, VA, CMS)

    • Completed Government Lien Intake Form
    • Agency-issued lien notification
    • TRICARE, VA, or MCRA-related billing
    • Conditional Payment Summary (CPS) or Demand Letter
    • Settlement details (if available)

Surrogacy Liens

    • Completed Surrogacy Lien Intake Form
    • Surrogacy contract or gestational carrier agreement
    • HIPAA form executed by the Surrogate
    • Insurance policy details or plan documents for the surrogate
    • Surrogate Compensation Details/Chronology
    • Any correspondence with health plans or their agents regarding the lien or reimbursement claim

Step 2: Submit Forms

Download the appropriate forms through the link/portal access provided upon execution of the Engagement Agreement. Once complete, submit all documentation in one of two ways:

Step 3: Schedule a Case Review

Before formally accepting any lien for handling, we conduct a brief case review to confirm:

  • All required documentation has been received
  • The matter aligns with our firm’s legal strategy
  • We can provide effective, tailored support to reduce the lien
  • Scope of retention

Please be advised that a consultation may take place either before or after the intake process, depending on the specifics of the case being referred.
